ARS § 14-5315 requires the filing of an annual report of guardian. Annual Report of Guardian
The statute provides that notice must be given to:
1. The ward.
2. The ward's conservator.
3. The ward's spouse or the ward's parents if the ward is not married.
4. A court appointed attorney for the ward.
5. Any other interested person who has filed a demand for notice with the court.
The template contains a proof of notice form, so no separate proof of notice is necessary.
If the guardian has mental health placement powers, at the same time that the Annual Report of Guardian is submitted, Rule 36 of the Rules of Probate Procedure requires that the guardian submit the physician's or psychologist's report pursuant to ARS § 14-5312.01(P).
