1. Retrieve the data in CSV format
2. Import the data into the Miller Trust Access Database
If client has ordered the trust online:
1. Prepare a pdf file
2. Burn the file to a CD
3. Print out 2 copies of the Miller Trust and Instructions
4. Prepare a Billing Matters Invoice
5. Make sure PB has signed the cover letter and engagement agreement
6. Send all of the above to the client by priority mail
1. The person requesting the Miller Trust should bring the following to the bank:
a. Drivers license or state id card
b. Social Security Number of the Patient
c. Copy of the Miller Trust for the Bank
1. Open a checking account in the name of the trust
2. It is better if this account does NOT earn interest
3. The least expensive senior checking account is the best
4. Trustee will be writing at least two checks monthly for the life of the patient - so trustee should order an ample supply of checks
5. Account should use the SSN of the patient - not the trustee
6. There is no need for a tax id for this trust because it qualifies as a grantor trust
7. Get a letter from the bank officer addressed to ALTCS informing ALTCS that the account was opened with a zero balance
