A Probate Information Sheet must be filed with any Petition requesting the appointment of a Personal Representative, Guardian and/or Conservator. This document is a confidential document pursuant to Rule 7 of the Arizona Rules of Probate Procedure and must be filed in a 9x12 sealed envelope. The Probate Information Sheet Cover Sheet is attached to the outside of the envelope. The Probate Information Sheet is placed inside the enveloped and then sealed.
